Dear Parents/ Guardians:
The Avonworth School District does not carry medical insurance for students but has arranged to offer student accident insurance at the parent/guardian’s expense. The United States Fire Insurance Company has been authorized to provide parents/guardians with the option to purchase K-12 Voluntary Student Accident Insurance coverage. There are two coverage options available for purchase.
You may obtain a brochure and application by clicking here. If you cannot access the online application, please contact your building secretary, and an application can be mailed to you.
If you wish to have your child insured, please complete the information requested on the enrollment form. Applications can be mailed directly to A-G Administrators LLC, PO Box 824936, Lock Box #824936, Philadelphia, PA 19182-4936.
